Well the government actually made that easy for us, There are two forms that can be downloaded, free, from the CMS website (Centers for Medicare and Medi-caid Services). They detail out a list of questions as to the patient's condition and situation to determine who is prime. It is called the MSPQ - The Medicare Secondary Payer Questionnaire.
Essentially there are circumstances under which someone else might be primary. There are many, so I recommend you check it out on the CMS website and search MSPQ
Some local FI's also have these tools online, so check with the fiscal intermediary that processes your Medicare claims for info too. But in a nutshell, these two documents will have all of the information you will need:
